Family Medical Leave Act

The Family and Medical Leave Act (FMLA) is a federal law that provides eligible employees entitlement to up to 12 work weeks of paid or unpaid leave during a consecutive 12 month period for the birth of the employee’s child, the placement with the employee of a child for adoption or foster care, a qualifying serious health condition of the employee, or a serious health condition of the employee’s child, spouse or parent. It also provides up to 26 weeks of paid or unpaid leave for military qualifying exigencies and care for injured or ill service members.

The Employee document is the FMLA application/Medical Certification form that the employee MUST have the medical provider complete and return to Human Resources in order for HR to determine if the employee is eligible for FMLA leave.
